Acceptance of Waiver

Acceptance: I HAVE READ, or have had read to me, the above CONDITIONS OF PARTICIPATION and FULLY UNDERSTAND the terms and conditions herein. I have received, or have waived my right to receive, independent legal advice with respect to all matters contemplated by these Conditions of Participation.

Parent / Guardian's consent of Dependents

Acceptance: I HEREBY CERTIFY THAT I FULLY AND UNCONDITIONALLY AGREE to completely indemnify TTC against all liability resulting from any injury or death that may be sustained by my son/daughter/ward, whilst participating in Tree Top Challenge. I HAVE READ AND UNDERSTAND the above TERMS AND CONDITIONS, ACKNOWLEDGEMENT, DECLARATION OF HEALTH, DECLARATION OF INDEMNITY, WARNING, and RELEASE contained within the adjoining CONDITIONS OF PARTICIPATION and agree that all shall apply to any Tree Top Challenge activity undertaken by my son/daughter/ward with TTC

Areas to note:

Our activites are in a natural environment with many paths that are un-graded, un-even, contain slippery surfaces when wet and can be a trip hazard. I understand it's my responsibility to walk with care and completely indemnify TTC against all liability resulting from any injury while walking in our activities

I am fully fit and able to follow all safety instructions of my supervisor and or sign boards out on course. I understand it is my responsibility to follow the instructions of my supervisor and failure to do so might cause injury to myself or others of which I am responsible. If I refuse to follow the instructions of TTC (signage or supervisors), I agree to TTC removing myself from the course to complete the induction again.

Health Check:

I do not suffer any illness, pre-existing injury, ailment or incapacity that may affect participation in such activities

I understand the activities may be physically demanding, requiring a moderate level of physical fitness and that the forces exerted on the body can activate or aggravate pre-existing physical injuries, conditions, or congenital defects. I understand that with any pre-existing conditions I should not participate and if I do, I completely indemnify TTC against all liability.

Parent / Guardian's Acceptance

As a parent / Guardian I understand the responsibilities outlined above and will ensure my children will follow the rules of TreeTop Challenge and accept responsibility if they don't.
